PROFILE, the financial aid application service of the College Board is a national, 100-year-old, not-for-profit membership association. Many of the College Board member colleges, universities, and scholarship programs use the information collected on PROFILE to help them award nonfederal student aid funds. The colleges to which you are applying may require you to complete and submit PROFILE.

Teacher Recommendation Request
This teacher recommendation request form should be completed for and given to each teacher to whom you have asked to write you a college recommendation. Attach it to a large (11×17) envelope and include a college teacher recommendation form for each college to which you are applying. Complete as much of the college recommendation form as you can. This will make the teacher’s work easier and result in getting your letter of recommendation and correctly completed forms to the admission’s office more quickly. This form will help you guidance counselor send out school reports and transcripts to the colleges to which you are applying. Include a School Report for each college to which you are applying.
