Teacher Recommendation Request

 This teacher recommendation request form should be completed for and given to each teacher to whom you have asked to write you a college recommendation. Attach it to a large (11×17) envelope and include a college teacher recommendation form for each college to which you are applying. Complete as much of the college recommendation form as you can. This will make the teacher’s work easier and result in getting your letter of recommendation and correctly completed forms to the admission’s office more quickly.
